For Nokia 3310 (I think others are pretty much the same):

Press Menu and select Option 5 (TONES)
Selection Option 5-4 (Composer)
Select an empty slot or one of your ringtones you no longer want

You then need to enter each of the codes in EIs message by entering the note (this is the letter in each grouping), the duration (this is the first number in the group before the note) and the octave (the number shown after the note. The notes are entered with the following keys:
1=c
2=d
3=e
4=f
5=g
6=a
7=b

The duration is set by pressing the 8 key (increase) or 9 key (decrease) until you get the right duration.
The octave is set by pressing the * key (the key under the 7).
A rest (shown as - in EI's message) is inserted with the 0 key (again use the 8 key to set the duration
The # key makes the note sharp (not used in this tune).

So the first few notes form EI's message are iserted as follows:

588 - should give 16g1 on display
1* - should give 16c2
3 - 16e2
59 - 8g2
etc.....

When you have finished entering press Options then Tempo and set to about 125 or 140 (I prefer 125). Then Save your tune and then go back to the Tones menu and select it as your ringtone.
